Request for Late Submission of Grades
If for any reason a Facilitator needs to submit grades after a class has already been closed by the Registrar, this form must be filled out to request approval to do so.
You can either complete the form below and submit it online OR download the form and complete it manually. If you complete the form and submit it online, the form will be automatically sent to the Registrar, Dean of Faculty and Dean of Students for their decision.
If you download the form, you must also print it, complete it, scan or photograph it, attach the scan / picture to an email and send it to all of:

- iTEE U Dean of Faculty (deanoffaculty@itee.university)
The decision of this committee will be final.
